Our housing options range from Supportive Apartments/Individual Housing Options to Adult Foster Care settings. In all RTI housing options we respect and honor the importance of a person’s home. Our goal is to provide quality homes with an appropriate individualized level of support to make that setting a success.
Apartment-Based Supportive Services
We offer a variety supportive apartment programs in the Twin Cities.Our apartment programs have a staff office onsite and Mental Health Workers available 24 Hours a day/7 days a week to respond to acute issues as they arise. We also provide each person with an Independent Living Skills team which will support and assist each resident in achieving the goals that the individual desires.

Community Residential Services in Homes
Residential Transitions, Inc. has variety of Adult Foster Care homes throughout Dakota and Hennepin County.In all of our homes RTI believes in a support model which fosters independence of the resident. Our homes vary in size and style. Beyond the traditional 4-bedroom home we also offer a few more unique settings such as one and two person homes.
